Information and documents to be provided by the person with the request:

A person wishing to be issued a Consent shall fill in the relevant Consent Application form available in the Consent Service and submit copies of the documents specified in the Consent Application Form through the DPA. The application for consent must contain the following information and must accompany the said request with the following documents : 1. Name, surname, place of residence, contact details of the natural person who submitted the application for Consent; the legal form, name, code and registered office of the legal person or other foreign organisation; 1.2. In cases where it is requested to transfer/pledge the lease right or transfer of buildings, the following must be submitted (indicated): 1.2.1. unique numbers of the buildings and/or equipment located in the state land plot (part thereof) to be transferred/pledged, unique numbers of the premises located in the building or the area of the part of the building to be transferred/pledged, when not the entire structure is transferred/pledged; 1.2.2. the date and number of the conclusion of the state land lease agreement; 1.2.3. the cadastral number of the state land plot; 1.2.4. a plan of the land plot in which the part of the land plot necessary for the operation of the transferred/pledged structures or installations (parts thereof) would be distinguished, the right of lease of which is requested to be transferred/pledged (if this part of the land plot cannot be established according to the plan of the land plot attached to the state land lease agreement), when the operation of the transferred/pledged structures or installations (parts thereof) does not require the entire leased state land plot; 1.2.5. a cadastral data file of the immovable property, confirming that the structure(s) in the state land plot has been newly built, reconstructed, overhauled or otherwise restructured (as provided for in Item 18 of the Cadastral Regulations), when the structure(s) were newly built, reconstructed, overly repaired or otherwise rearranged in the leased state land plot. Newly built or under construction, reconstructed, overhauled or otherwise reconstructed structure(s) must be registered in the Real Estate Cadastre and the Real Estate Register. 1.3. In cases where it is requested to temporarily use state land for agricultural activities for remuneration, the following must be indicated: 1.3.1. the cadastral number of the state land plot or the area of the unformed state land in which it is desired to use the state land temporarily for remuneration. This can be done manually by drawing the planned area of state land or a plot of state land (part thereof) with the help of the tools on the environmental map of the Consent Service, or by marking the coordinates of the centre of the planned temporary use area of state land or a state land plot (part thereof), or by uploading a drawing of the planned temporary use of the area of state land or a plot of state land (part thereof) in *.dwg format; 1.3.2. the area of the state land desired to be used; 1.3.3. Consent Term; 1.3.4. the basis on which consent is requested; 1.3.5. Other information and documents related to the application. 1.4. In cases where the management of the coast of a water body is requested, the following shall be indicated: 1.4.1. the cadastral number of the state land plot or the area bordering on the water body; 1.4.2. Term of consent; 1.4.3. the basis on which the Consent is requested; 1.4.4. Other information and documents related to the application. 1.5. In cases where it is requested to issue a Consent for the establishment of the territory of special conditions of land use, the following must be indicated: 1.5.1. the cadastral number of the state land plot or the area of the unformed state land in which it is requested to establish the territory due to the special conditions of land use. This can be done manually with the help of the tools on the environmental map of the Consent Service by plotting the territory where the special condition or the coordinates of the centre of the state land plot (part thereof) is requested, or by uploading a drawing of the area of the land area or the state land plot (part thereof) in *.dwg format of the planned territory where it is desired to establish the special condition; 1.5.2. Term of consent; 1.5.3. the basis on which the Consent is requested; 1.5.4. Other information and documents related to the application. 1.6. In cases where sublease of a state land plot (part thereof) is requested, the following must be submitted (indicated): 1.6.1. a copy of the contract granting the lease or other right of use of the buildings or installations (part thereof) located in the leased state land plot concluded for a period not exceeding 5 years; 1.6.2. the date and number of the conclusion of the state land lease agreement; 1.6.3. the cadastral number of the state land plot; 2. a document confirming representation or a certified copy of this document (if the Request for Consent is submitted by a person authorised by the person requesting the Consent to be issued).
